E-signing for clinics and allied health practices.
Physiotherapists, osteopaths, psychologists, dental and medical practices all ask patients to fill in and sign forms before the first appointment. Sending those forms ahead of time means a shorter wait in reception and fewer clipboards. This guide covers the workflow side only: how to send your own forms and get them back.
Intake forms before the first visit
Turn your new patient form into a template. Use text fields for contact details and the GP's name, tick boxes for the history questions you ask, and Yes/No choices where a single answer is needed. Mark the fields that must be completed so the form cannot come back half done.
Send it when the booking is made. Patients complete it on their phone at home, and the signed copy is waiting when they arrive.
Consent forms for a particular treatment
Some treatments come with their own information sheet and consent form, written by your practice. Put both in one envelope so the patient reads the information and signs the consent in one sitting. Switch on document summaries if you want them to see a plain-language overview first, and remember that the summary sits alongside your form, never instead of it.
If a patient has a question, they can decline with a note rather than sign. Your clinician sees the note and can follow up before the appointment.
When someone else signs for the patient
For children, a parent or guardian is the signer. Where both parents should sign, add each of them and let them sign at once. For older patients supported by family, add the person holding authority as the signer and copy the patient in if that suits your practice.
Practice paperwork beyond patients
The same workspace handles your own business documents: agreements with contractors who rent a treatment room, locum arrangements, supplier agreements and staff employment paperwork. Practice managers can be admins, and each clinician can have their own login with access to the templates they use.
- Room rental and contractor agreements with independent practitioners.
- Locum and relief arrangements.
- New staff agreements and policy acknowledgements.
A record you can find later
Each envelope keeps its audit trail of views and signatures, with names, email addresses, times and IP addresses. The patient receives the completed form by email with a completion certificate, so they have their own copy too.
Common questions
- Can a patient fill in the form on their phone?
- Yes. Everything works in a phone browser, including tick boxes, text fields and signing.
- Can we collect a referral letter with the form?
- Yes. Add an upload field and the patient attaches the file while completing the form.
- Do patients need to create a login?
- No. They follow the link in the email and sign without an account.
- Can reception see who hasn't completed their forms?
- Yes. The envelope list shows who has opened and signed, and reception can send a reminder.
