E-signing for gyms, fitness studios and personal trainers.
The first visit to a gym or studio often starts with a clipboard: a membership form, a health questionnaire and a page of house rules, filled in while the new member's class is about to begin. Sending those documents ahead of time gives members a calmer start and gives your front desk a complete record without any scanning.
Send the joining pack before day one
When someone books an intro session or buys a membership, send their joining pack straight away. One envelope can hold the membership agreement, your house rules and the health questionnaire, so the member opens a single link and works through each one. Most people complete it on their phone the same evening.
Use a template with a Member role. Your staff only type the name and email, and every field is already in place.
A health questionnaire people actually finish
Yes or No choices suit the questions trainers care about: recent injuries, heart conditions, pregnancy, medication. Mark each one as required so the form cannot come back half done. Add a text box after the questions so the member can explain any Yes answer in their own words, and a date field for when they completed it.
Because the answers come back as a signed document, your trainer can read them before the first session rather than after it.
- Required Yes or No questions for each health topic.
- A text box for detail on any Yes answer.
- A tick box confirming the member will tell you if things change.
- Signature and date at the end.
Personal training packages and studio hire
A block of personal training sessions, a six-week challenge or a casual room hire for a yoga teacher each has its own terms. Keep a separate template for each so the right wording goes out every time. For a teacher hiring your studio, send in order: they sign first, then the owner countersigns.
Changes, holds and cancellations
Members ask to pause their membership when they travel, change their payment day or cancel. A short form with tick boxes for the type of change and a date for when it starts gives you a clear, signed request each time. Copy in whoever handles direct debits so they act on the same document.
If you only need the member to confirm they have seen updated house rules, send a review-only envelope. They acknowledge receipt rather than signing.
Seeing who is still to complete
Status tracking shows which new members have opened their pack and which have not. Automatic reminders do the chasing, so reception staff are not following people up at the door. Each completed envelope carries an audit trail with names, emails and times.
Common questions
- Can a parent sign for a junior member?
- Yes. Send the envelope to the parent or caregiver as the signer and include the young person's details as text fields.
- Can members fill it in on their phone?
- Yes. The link opens in any phone browser with nothing to install.
- Can trainers see the health answers?
- Add your trainers to the workspace or copy them in, and they receive the completed document.
